PDGC Membership Form

This agreement releases the Penticton Disc Golf Club (PDGC) and the British Columbia Disc Sports (BCDS) from all liability relating to injuries that may occur during disc golf tournaments and BCDS-sanctioned events. By checking this box, I agree to hold the Penticton Disc Golf Club and BCDS entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether the injuries are caused by negligence.

I also acknowledge the risks involved in disc golf. By checking this box, I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

By checking this box, I forfeit all right to bring a suit against the Penticton Disc Golf Club and/or the BCDS for any reason. In return, I will receive the ability to participate in Penticton Disc Golf Club and BCDS events. I will also make every effort to obey safety precautions. I will ask for clarification when needed.

The Penticton Disc Golf Club is committed to the protection of your personal information. The personal information that you provide in this membership application is used by the Penticton Disc Golf Club to communicate with you and facilitate your participation in club activities. Your name and address are disclosed to BC Disc Sports to satisfy club affiliation and insurance requirements. Your name, gender, and the division you compete in may be published in association with your participation in club events (such as the posting of the tournament results or league event that you participated in). Application for Penticton Disc Golf Club membership requires that you consent to the above uses of the personal information you provide in this application form. If you are the legal guardian of the application and/or you are disclosing personal information of family members, you acknowledge having received their prior consent to your representations in this regard.

Do you give the Penticton Disc Golf Club Board consent to send  you notification emails for the purpose of membership renewals, annual general meetings, and other club related events (tournaments, maintenance days, etc)
